From January to December 2016 we worked with South Ayrshire Health and Social Care Partnership on the final Pilotlight pathway, Living Well with Self-directed Support. This project was funded by the Scottish Government under the National Strategy for Self-directed Support ‘Building the capacity of support organisations’.
Key outcomes of this project are:
- Systems and attitudes identified that hinder access to self-directed support for individuals diagnosed with younger onset dementia and their carers and all stakeholders encouraged to overcome these barriers
- A co-designed pathway to self-directed support for people with younger onset dementia, underpinned by best practice models and tools
- Improved relationships across the partnership and with providers to deliver self-directed support information and support services to individuals and their carers
- Learning gained across the life of the project around creative design methods in order to support individuals and their carers to exercise maximum choice and control over their self-directed support.
