From April 2015 to April 2016 we worked with East Renfrewshire Health and Social Care Partnership on Ageing Well with Self-directed Support. This project was funded by the Scottish Government under the National Strategy for Self-directed Support ‘Building the capacity of support organisations’.
In this pathway we worked with older people, carers, health and social care practioners and independent providers of support, information and advocacy to explore the opportunities that self-directed support offers to plan creatively for transitions with older people living in the community.
On 21st April 2016, the Ageing Well with Self-directed Support co-design team in East Renfrewshire presented their work so far to a group of stakeholders. You can view their presentation below.
