Who is it for?
The SDS Mapping Tool is for anyone supporting a person through their SDS journey.
What does it do?
The SDS Mapping Tool maps all the people and steps involved in an individual’s journey to SDS. It is designed to help the person know who is involved, what their roles are, what each step will be and when it will take place so that they can exercise as much choice and control as possible over the process.
How can you use it?
The SDS Mapping Tool is designed to be hung on a wall as a calendar. The key or main supporter should take the lead in supporting the person to request photographs and contact details for each person and recording each step (meetings, telephone calls etc.) as they are arranged.
