My Living Well Pack is for anyone newly diagnosed with dementia and their supporters.
What does it do?
My Living Well Pack provides prompts for information to be sought and discussed about the dementia pathway and a place to store it for easy reference.
How can you use it?
My Living Well Pack can downloaded and made up as shown in the photo instructions. It can be used by the person with dementia and shared with others as they see appropriate. Providers of post diagnostic support can alert people newly diagnosed with dementia and their supporters to the availability of the pack and use it as a framework for their support.
