At what stage of the design process is the tool most useful? 
What does it do? 

The inclusion form identifies any support requirements of participants to enable everyone to participate equally. Facilitators can then respond by making the necessary adaptations to the workshop design and environment.

How do you use it? 

Before undertaking any design workshops, meet participants on a one to one basis and complete the checklist together. In designing the workshops, use the checklists to ensure the needs of all participants are catered for.

You may find it helpful to revisit the checklist part the way through the co-design process - participants may be more willing to give information about their additional support needs once they get to know you and you may observe additional support needs not already catered for during the workshops.
